What Causes Painful And Hard Bowel Movements Despite Taking Exlax?

My 20 month old granddaughter is having severe pain with bowel movements. The stool is extremely large. She has a good diet of fruits,vegetables,and plenty of fluid. Her dr said to cut back on milk which happened and still no change she screams in pain but the response we have been getting is give her some exlax. I don’t want this young child to experience this pain or become dependent on stool softeners. What can be causing this please help!
