For the last several weeks I have a really bad ache in my rectum. It starts out kind of dull ache then will get really bad after about 15 minutes and stays bad for about 15 more minutes then it goes away. I have tried over the counter medication for hemorrhoids and cold cloths but nothing seems to help. I have not had any bloody stools.

It is possible that you have an anal fissure. Here are a few things that may help:
1) Increase your fluid consumption. 2) Increase your roughage/fiber consumption. This will help you pass stools without straining. 3) You could take a laxative which is also available over-the-counter. 4) If this doesn't work, you could visit your clinician/surgeon to evaluate the same.
