What Causes Pain In The Pelvis And Testicles During A Bowel Movement?

hi, a few months ago i bump appeared in my anus.I was in a lot of pain and took a hot bath.The lump exploded and blood came out.Then about three weeks later i had to spend quite a while seated in order to do some school work.There was a moment where my legs started burning,had testicular pain in both testicles,pelvic pain and anal pain(stabbings,burning,feeling of a foreing object,etc).I went to the er,they did a sacrum,thoraxic,pelvic,abdomnal mri and found nothing,They also did x-rays to my testicles and they didn't found anything.I have been in constant pain for 3-4 months now,and when i have a bowel movement,my right pelvis,right above my right testicle hurts,along with my leg and my lower back on that side.
?Right now the pain is almost excrusiating, i wake up with my legs,butt,perineum burning and with lots of spams all over my body.
Last but not least,i bought a hemorhoid cream and every time i put on,on my rectum right side,i feel a lot of pain but then it relieves my testicular,pelvic and lower back pain from my right side.
any idea what this could be?

* This lump is anal pathology with most possible peri anal abscess or
fistula extending into ischiorectal fossa with referred pain in form of
testicular discomfort .

* Clinical evaluation with procto anoscopy is a must for the diagnosis .
