What Causes Numbness On Face When Diagnosed With Hypothyroidism?

2002 - after a fast weight gain I was diagnosed with Hypothyroidism. I have gained 52kg in 3 months (from 65 to 117kg). I was hospitalized for 2 months and took Euthyrox 50mg for 2 months, then out of the hospital they gave me Euthyrox 25mg for 1 year. The weight gain stopped but I was not able to lose more than 20kg since then. I started feeling depressed, stopped Euthyrox and didn t see any doctors untill 2015.            2015 - I slipped on the stairs, the nosing of the stairs hit my sacral region. While I was trying to get my balance back my left leg came under me and i double-fractured my big toe. I felt a huge pain going from my toes to my head on the left side, trough my leg, arm, face till my head. But it was only for 2 seconds. I sat where I fell for 10 minutes, trying to catch my breath then I was able to move. After 10 minutes I felt dizzy, nauseous and I suddenly lost my consciouness. There was someone with me and she told me i was unconscious for 1 minute and that when I fell I hit the wall next to me with my head. When I woke up after 1 minute, I trid to stand but I couldnt control my legs. It took my 5 minutes to be able to stand up. For the next 20-30 minutes, I was not able to talk correctly andI still had that nausea feeling. I knew very well what I wanted to say but the words coming out of my mouth were not in the right order. I was taken to the hospital. Had a CT scan that revealed nothing, 2 hours of IV fluids and then they released me. Because I was confused and scared of a head injury,I totally forgot to tell the doctors about my big toe problem. For 1 month I was ok, but after that, I started having severe migraines, to the point where everything was turning black and I couldnt walk. Also, my left leg started to swell from my foot to my knee. Went to the doctor, he took an X-ray and told me My big toe has 2 fractures. But he didnt put my leg in a cast or any treatment. He just said I shouldnt sit down more than 15-20 minutes, to reduce the swelling. Then I went to a neurologist and took an MRI. Also an endocrinologist, without any blood tests(!?), said I now have Hashimoto thyroiditis. MRI results: -motor deficits: no -pathologic reflexes: no -Romberg: the patient is faling to the right, left and back -coordination disorders: no -cranial nerves disorder: no -head MRI: solitary focal lesion in theright frontal white matter, with no perilesional edema. Treatment: -Biobil 40mg -Cebrium -Betaserc -Ca, Mg, Zn 5.02.2017 - I wake up in the morning, feeling my face numb on the left side. I went to the ER and after some tests, they decide to hospitalize me. Diagnose: progressive left hemifacial atrophy. Started on the left side, forhead to cheek. Arterial blood pressure=120/80 mmHg Heart rate=70 bpm Glycemia=103mg/dl Thermal and tactile hypesthesia Romberg s test: positive Platnar reflex: positive Babinsky: positive right, negative left Blood analysis results (gonna write down only the wrong ones): -total serum cholesterol: 219.00mg (normal values 120-200) -triglycerides: 181mg (normal values 40-150) -eosinophil: 5.70% (normal values 0-5) -hematocrit: 29.12% (normal values 36-48) -hemoglobin: 9.02g (normal values 11.70-15.00) -MCH: 22.62pg (normal values 27-32) -MCHC: 30.92g (normal values 32-36) -MCV: 73.12fL (normal values 80-100) -MPV: 10.70fL (normal values 7.40-10.40) -RBC: 3.98 (normal values 4.20-5.60) -RDW: 17.91 (normal values 11.60-14.40) -VSH: 36mm (normal values 2.00-25.00) - PT: 14.70 (normal values 11.00-14.50) CT scan: right frontal subcortical lesion, measuring 0.7cm. Stayed in the hospital for 3 days with only a CT done. It was taking too long to have all the investigations done in the hospital so they released me to try to have them done on my own, faster. Repeated blood analysis on 14.03.2017: -VSH: 52mm (normal values 2-20) -HB: 8.8 (normal values 11.6-15.5) -HCT: 29.6 (normal values 33.5-46) -MCV: 74.2 (normal values 78-100) -MCH: 22.1 (normal values 26.5-34) -MCHC: 29.7 (normal values 31-37) -RDW: 18.2 (normal values 10-16.5) -serum iron: 19.6 (normal values 60-180) -B12 vitamin: -A-TPO: 2386 Ui/ml (normal values Evoked potentials results: normal. EMG test: normal. Since 5.02.2017 till today, I have seen 2 hematologists who only saw a small problem with my iron levels, but the rest of the blood test results didnt raise any signs of concern for them. 5 neurologists who put my under observation for multiple sclerosis. No one saw anything wrong with my A-TPO levels, B12 deficiency, VSH or any other of the blood tests mentioned above. With no diagnosis they put me on Medrol for 6 days (decreasing from 6 pills at once in the first day to 1 pill in the 6-th day) and Gabaran 300mg for 1 month (1 pill/day). For the last 3 weeks the face numbness covered my whole left side, i feel my tongue swollen but it looks normal, when I touch my face I feel the presure but I cant tell the texture, I cant feel the taste on the left side of my tongue and I feel something like an electrical impulse on the numb side. Also, another brain MRI showed 2 nonspecific demyelinating lesions (0.4mm and 0.8mm) on the supratentorial white matter and ethmoiditis. I feel pain in my right arm, starting from my shoulder all the way down to my fingers. Painkillers ease the pain the the arm for a little time but not the one in my hand. I feel burns and needels in my hand, mostly in my index finger. I cant hold my hand down cos i feel a huge pressure and pain in my hand and fingers, like the hand whould get swollen but its doesnt. I can not use my index finger, i cant bend it and when i touch it, even the skin hurts like its burned. During the EMG, the muscles in my index finger contracted but I couldnt feel the electric pulse in it. A 6-th neurologist sent me to a rheumatologist for further investigations, thinking I have Lupus disease or Guillain-Bare Syndrome. I need help. I m a 33 yers old and I cant handle the pain and numbness anymore. It s starting to affect me a lot and, after seeing all those doctors, with no results, Im starting to think I ll just have to get used to all the pain... PS: Excuse my English, I m not native, I m from Romania.

General Surgeon 's  Response
Hi,

As per my surgical experience, the hypothyroidism can cause facial puffiness or so, but not numbness.

The numbness and weakness sound to be secondary to facial nerve lesion, calcium disorders or else, which has to be evaluated thoroughly.

Hope I have answered your query. Let me know if I can assist you further.

Regards,
Dr. Bhagyesh V. Patel,
