What Causes Nausea Along With Diarrhea And Chest Pain?

Started feeling nauseous and had diarrhea with no fever for 3 days, went to urgent care and was told I had stomach flu. I have been a cigarette smoker for 20 years and woke up on day 5 of illness with no urge to smoke, like a switch flipped in the back of my brain, I also stopped drinking coffee and smoking marijuana with no desire to continue. I started having chest pains and anxiety, after several er visits I was diagnosed with GERD and anxiety. My mental health started to get to a dark place and went to er with suicidal ideations, was diagnosed as depressed. I refused the Ativan and Zoloft but agreed to take Omeprazole. My symptoms seemed to linger, I had rapid weight loss and my mental state was suffering, i thought maybe I had blood sugar issues, anemia, or even premenopausal (I’m a 35 year old woman), my GP assured me that I’m fine and all my blood work is normal, I requested vitamin deficiency testing and just received the results as borderline deficient. I have been tested for everything below the neck but I wonder if it’s a brain thing??? I have had no injury to the brain but I feel like a different person, like a switch flipped, like my brain is sick, am I crazy?? Currently taking vitamin d and b12 supplements and stopped the PPI meds....

I had seen your detailed medical history and understood all the relevant concerns. I can understand your discomfort and anxiety.

First of all i need to clarify some important aspects regarding your symptoms. The symptoms like nausea and diarrhea associated with chest pain are most commonly seen in GIT (gastrointestinal tract) diseases or any GIT infections.

As you mentioned you were a smoker for 20 years, smoking effects not only lungs but also GIT. It can cause damage to the epithelium of intestines. As your GIT became hypersensitive to smoking you were avoiding. Mostly it might not be related to your brain. As your GIT was effected your digestion and absorption capacity decreases, so there might be weight loss. It can lead to variation of sugar levels also.

By considering all the aspects of your health the most probable cause of your symptoms might be damage of epithelium or irritable bowel syndrome. That was the reason you were repeatedly getting GIT problems (stomach flu, GERD etc). Irritable bowel syndrome occurs in people taking more of stress in life.

You can confirm the GIT condition and exclude all other conditions by following investigations:
1. Endoscopy or colonoscopy
2. Gastric function tests
3. MRI brain to exclude brain defects

Coming to treatment part
1. vitamin tablet is a good choice
2. better to have small frequent meals
3. taking less spicy food and avoiding smoking
4. Omeprazole can be continued and dicyclomine can be taken
5. Drinking plenty of water and relieve stress factors

As you are not too old, the epithelium will be regained. Go for investigations and act accordingly. If any abnormal thing is found you can consult us at any time.
