What Causes Lower Back Spasms With Pain And Fatigue?

Lower back spasms, pain, chills, fever, and fatigue. Fever at 104 now. Contraction like spasms ever 5-8 hours. Three weeks of pain. Doc keeps saying its just a back spasm but the muscle relax, pain med and anti immflam aren t fixing anything and barely keeping my husband functional. We have been to doc several times with little to no serious help.
