What Causes Light Bleeding From Rectum During Bowel Movement?

I had a hysterectomy 5 yrs ago -Lately I have noticed when I put underwera on by the end of the day they are discolored- I do have bowel movements alot because of my medicine and sometimes there is light bleeding from my rectum on the paper very little
