What Causes Itching In The Nipples?

I have a fifteen year old that has been complaining of itchy nipples for over a month. We tried different lotions, Vaseline, but not much relief. Today she removed her white bra and was terrified by finding discharge in her bra. I need to say that she is large chested for her age. I have never experienced anything like this, so not sure what to do. She just had a menstrual cycle, and is on birth control. Advice?
