What Causes Itching And Burning With Urination?

My daughter is now almost 8. Have seen about 9 doctors, many specialties for 3 years until my daughter was dx d with lichen sclerosus. Another derm, did bx of vulva area close to introitus and results came back Spongiotic Dermatitis with rare eosinophils. She was given clobetasol oint to use bid. The med made her skin worse and did not help her initial problem of pain of introitus. She has burning with urination, itching, burning. She cannot sleep at night and has no relief in the day. She is very specific about where the pain is precisely on introitus and they appear like very tiny blisters. This area is what seems to cause her stabbing pain. Can you please help?

Hello and Welcome to ‘Ask A Doctor’ service.
I have reviewed your query and here is my advice.

Cause of itching and burning is recurrent infection most likely.

I would like to recommend some steps for her better outcomes.
• Keep genitals clean, with an antiseptic liquid, and dry.
• Allow her plenty of liquids by mouth as much as possible.
• In our clinic, we recommend urine culture and antibiotics only accordingly with supportive urinary alkalizers, anti-inflammatory agents, antihistaminics, cranberry syrup, and vitamin c supplements to get better and have faster positive outcome.
• Make her wear loose, cotton, and clean undergarments.
• Long-term application of steroids creams make her dependent and unresponsive, so I recommend to use aloe vera gel for best results.
• Avoid exposure to excess heat.
• Avoid more oily or spicy or non-vegetarian stuff.
• Make her practice to use washroom as and when required immediately. Do not withhold urine for long periods.
• Daily bath with Epsom salt added in lukewarm water and lavender oil give tremendous outcomes in 30 days.
• Keep her more ambulatory with walking, exercises, and yoga.

Hope I have answered your query. Let me know if I can assist you further.

Regards,
Dr. Bhagyesh V. Patel
