What Causes Internal Bleeding Along With Low Haemoglobin Levels?

my question is on behalf of my 37yr old male cousin. He has been told he is internally bleeding, has had one blood transfusion already & has vertigo & pain so bad he can not work. They keep sending him home & he waits weeks for returned phone calls to book appointments for tests. So far they cannot find anything wrong with him, colonoscopy found nothing but blood work showed that his hemoglobin is low. He also has very dark stool. Now he was a smoker but quit about 2months ago, & he ha a pinched nerve in his shoulder so he was taking naproxen regularily but has been instructed to stop about a month ago when this all began. What could be causing this problem???

I have understand your concern

most probable reason for his problem is upper GI bleed only , because dark stool indicates only upper GI bleed and colonoscopy also Normal.

mostly naprosen induced ulcer bleed or some other cause.
I can suggest him to get done upper GI endoscopy , if it is already done , he can undergo capsule endoscopy which can find out bleeding spot where colonoscopy and endoscopy can't reach.

one more scan is there, that is RBC scintigraphy or bleeders scan , all these are costly

meanwhile if his HB is less than 8 gms , he needs blood transfusion.

somatostatin infusion also helps in controlling bleeding
