What Causes Intermenstrual Bleeding?

Had laparoscopic surgery for endometriosis in 2008, menstrual flow, clots, cramps gotten worse since child #3, in 2014. My cycle is like clockwork. Finished reg period, and 10 days later I am experiencing thin, bright red vaginal bleeding, no pain, minimal small clots lasting for the past 5 days. I have not been sexually active in more than 2 months.

Inter-menstrual bleeding is likely due to some kind of anatomical pathologies in the genital tract like polyps, fibroid etc. Pelvic/vaginal examination and pelvic ultrasound are required for initial screening.
