What Causes Inter Menstrual Vaginal Bleeding?

I turned 50 in April. I have never had issues with my periods, no major cramping, heavy bleeding etc . Just a normal 3 to 4 day period. Last Nov I had a two week period, then normal again. Last month I had a two week period, off for two weeks, back on and two weeks is almost here and no sign of stopping. What s going on, why, what can I do? Is it pre menopause or something else to be concerned about. It is frustrating and starting to concern me.
