What Causes Heartburn?

Hi, I am a 36 female. I have had two children naturally. I have no known allergies, and take a multivitamin daily. I am not overweight, eat a healthy diet, exercise 5x week. I do not smoke and only drink alcohol occasionally. Now for my question. I have some concerning symptoms. 1. Always fatigued. 2. Sharp to gnawing pain in my abdomen right under my breast bone that goes through to my back. I don't always have it, but had it occur again the morning after having had 2 1/2 glasses of wine at a party. ( a rarity for me) tenderness to palpation all across the top quadrants of my abd. And a palpable and tender bulge in the upper right quadrant. I now have had heartburn everyday after eating for one week. I never get heartburn either. What the heck is going on with me? I'm a nurse and tend to put things off, but I'm getting a little worried. Please advise? Is it time to see a GI doc?

Thanks for your query.
Noted the history and understood your concerns.
Since you are a nurse, it should have been easy for you to have a proper possible diagnosis and to take an opinion of a Doctor or a Gastroenterologist.
Well, heartburn is suggestive of GERD and pain in the upper abdomen also may be due to acidity related things.
You can try the following:
Get a PPI like Omeprazole.
Take antacids when on empty stomach.
Domperidone for regulation of motility and not allowing the stomach contents to reflux into the esophagus to avoid heartburn.
Get an ultrasonography of the whole abdomen to get the exact cause of the swelling whether related to the gall bladder or liver and get further tests of blood for CBC, liver and other functions.
Upper GI endoscopy will help to clear the diagnosis further.
IF you still do not get a complete relief, it is better to take an opinion of your Doctor and get a reference for a Gastroenterologist for clinical evaluation, further investigations and a proper management.
Control of stress and anxiety is very important as it is a part of the lifestyle and your job profile.
