What Causes Hard Knot Near Spine Behind Skull, Migraines And Neck Stiffness?

I have a hard knot very close to my spine on the left side right under the back of my skull. I have been having migraines and its painful with stiffness to my neck. Roughly the size of a quarter in diameter. Under the skin with no sores or redness. Can you tell me what this is and if there should be any concern?
