What Causes Fullness Of Stomach Even If Not Eaten Anything?

I am a 48 year old female i have been over wieght my entire life..and i have struggled with many crash diets, i lose and i gain it all back...i smoke, i get hardly any exercise at all, I am active in my daily life. Altho lately i have become sluggish..i have had high blood pressure and high cholesterol for many years...i find myself short of breath, i have heart palpitations, i sweat at night whe i sleep...i have a feeling of fullness all the time ever though i havent eaten..what could this be??
