What Causes Facial Edema After Having Appendectomy?

Hi, My cousin had an appendectomy about 2 days ago and he has not passed flatus yet. The doctor said that the appendix had ruptured and there were significant pus already in the abdomen. I was also found that there were significant small fluids already in the lungs upon admission. last night, we noticed that he have facial swelling and kinda have shortness of breath. What could be the reason of the facial edema? Thank you so much.

i appreciate your concern

your friend might have been suffering from multi organ dysfunction due to spread of infection
he has fluid in lung and facial swelling may appears in a kidney disease

I.would advise for your friend to get following lab investigation besides clinical examination
hemoglobin Complete blood count ESR liver function test kidney function test like blood urea serum creatinine and BUN

he needs hospitalisation and and a course of intravenous antibiotics and supportive care
