What Causes Epididymitis After Unprotected Intercourse?

Hi Doctors I need some help... I have unprotected sex with a girl on 7 May. on 9, May, I suffered epididymis inflammation (right side) and on/ about 13 May I have fever for a week, Doctor said epididymis inflammation it is not related to HIV. After treatment for 3-4 weeks with antibiotics,my right testical is fine but my left testical starts pain. I took first HIV 1/2 ag/ab combo test on 2 June - result is negative, in June - July, I have got some various symptoms likes, rashes on my neck & body, white spots on my skins, fatigue, blurred vision, weight loss for almost 10% of my body weight, white bumps on lips, mild mouth ulcer,...both seems to be HIV symptoms. On 9 Jul (9 weeks after exposure) I took another HIV ag/ab test and the result is also negative. Can I consider the test at 9 weeks conclusive? Will there have any other types of HIV which is not being tested in my report? During these 2/3 months, I saw some of my colleagues, families seems having same symptoms likes rashes, cough, irritated skins...if I am infected, am I very contagious in acute infection stage ? Is my saliva, sweat contagious to others? Because some people in my office, I didn t touch them but they are having same symptoms as mine. The only thing we share is dishes, toilet, and some office tools only. I m soooo worried on this as seems this disease is very contagious in environment.

Hi,

This type of pain and location is suggestive of epididymitis which is inflammation of part of the testicle and this not uncommon in males especially in young age. This is usually treated with 10 days of antibiotics, usually ciprofloxacin and there are no permanent consequences.

Masturbation and intercourse should be avoided till pain persist, if possible since I may prolong inflammation course. Some more serious conditions such as tumors or torsion are rare but in doubtful cases, you should do Doppler ultrasound or scintigraphy. In this particular case, I don’t think this is required and this should improve on antibiotic therapy.
