What Causes Enlarged Lymph Nodes Above Collar Bone?

I have had a knot in my lymph nodes on the right side by my collar bone area for nearly a year. it started out as small as a pen head and now it is the size of a quarter. should i be concerned an what should i do? i do not have insurance so feel hopeless.

I really appreciate your concern, if this is selling and if this has been increased in size then it need to be brought to investigation because it could be due to some infection if it is lymph node for the swelling "Tab Ibuprofen 400 mg" three times in day can be tried, hope this information helps, take care and have a nice day.
