What Causes Elevated WBC, MCV, Neutrophils And Monocytes In A Postpartum Mother?

Im not sure...but I hope so :-) I was looking at the CBC (no differential) of a postpartum mum. The abnormals were elevated WBC (to be expected), MCV, Neut, and Mono. I was wondering what the reason would be for the elevated MCV, Neut and Monocytes...possibly infection? or is this expected in a postpartum mum?

Hi
Dr Purushottam welcomes you to HCM virtual clinic.
I think I have understood your concern.
WBC count upto 12000/cmm is normal in post partum.
In that if Neutrophils are raised- above 75 to 80% it is suggestive of infective focus.
Also we need to consider clinical condition of the patient, if she has foul smelling discharge or fever or breast engorgement or breast abscess.
I hope my answer helps you.

OBGYN Dr. Purushottam Neurgaonkar's  Response
Hi
Dr. Purushottam welcomes you to HCM virtual clinic!
Thanks for consulting at my virtual clinic. I have carefully gone through your case, and I think I have understood your concern. I will try to address your medical concerns and would suggest you the best of the available treatment options.
1] Please do not panic.
2] In post-partum woman, if total WBC counts are above 12000, with Neutrophil cells are above 75%, and she is experiencing signs of infection , then one can call it to be having infection.
3] Along with lab reports, we have to consider if she has pain in lower abdomen, to see if uterus is shrinking in size, any pain or discharge from episiotomy or LSCS site, or foul smelling vaginal discharge, burning urination; any of these will be suggestive of post-partum infection.
4] I will suggest to have close watch on temperature, every 4 hours.
4] Tab. CALPOL 500mg as needed to get down fever.
5] Appropriate antibiotics can be started if required.
I hope my answer helps you.
Thanks.
Wish you great health.
