What Causes Elevated BP And Cholesterol Levels?

I did a health screening at work. My total cholesterol is 219 with my LDL at 110, I also have high blood pressure. I am 5 2 and I only weigh 115lbs. I have family hx on both sides of my family. Should I be worried? And if so, what can I do to lower it? I exercise 3 to 4 times a week, and I eat a very healthy diet. Any suggestions would be appreciated. Thank you.
