What Causes Dizzy Spells And Drop In Oxygen Levels On Exertion?
So my grandma has Pulmonary Hypertension and is currently on Veletri which is a medicine that is not very well known. Her baseline O2 stat is usually around 85% with nasal cannula on 3 Liters of oxygen. She currently right now had a dizzy spell and now her 02 dropped down to 72 while laying down. She sat up and put her cpap on and now it s at 82 but anytime she moves or talks it drops. What could be going on? Should I take her into the ER. The only problem with going into the ER around here is nobody is used to dealing with her continuous infusion of this med into her heart so they usually won t deal with her here in Saginaw we have to drive an hour and a half away to Ann Arbor. What should I do any suggestions?
* The inability to maintain oxygen saturation in all positions indicate certain ventilation perfusion mismatch at the pulmonary alveolar interface levels with capillaries draining the oxygenated blood.
* In my clinic, I do recommend
- a thorough clinical examination
- pulse oxymetry reading
- EKG
- portable x-ray chest in both recumbent and semi reclining position with decubitus or lateral shooting view plate
- all blood gas analysis with complete hemogram
Later on further management plan should be figured out.
