What Causes Difficulty In Urinating?

I am 30 years old, single, I started noticing some difficulty urinating after I masturbate, I did not pay attention to this since it was temporary. I had girlfriend last year thin we stopped seeing each other, after that I started masturbating again 1 to 2 times everyday, 4 months ago I met a girl and we had sex, I only lasted few seconds which never happened before, I was devastated, however few days later I had gonorrhea so I stopped seeing her. I was treated with ceftriaxone and azithromycin. and symptoms resolved. I met another girl and I also lasted for few seconds less than 10 seconds. this problem continued, I did not know what to do to treat premature ejaculation so I stopped masturbating because I also noticed the amount of my semen is becoming less. I met my current girlfriend and we are getting engaged next month but we did not have any sexual encounter yet. for 2 month now I have more difficulty initiating urination, and I feel numbness in the tip of my penis and scrotum. I masturbate every 3 days to relieve the pressure, but I only last for seconds when I do which was never the case before, even if I masturbate immediately for the second time I last longer but just for couple minutes. I saw a urologist because I thought I have some sort of prostatitis, I told him about my symptoms and that some times I see white clouds in my urine, he did rectal exam and he said the size is normal but it slightly swollen or boggy, urine analysis was clear. he gave me 5 day course of ciprofloxacin and naproxen ( which I finished last month) , he said even f its prostatitis we do not treat for 4 weeks now, because studies showed no benefit of prolonged antibiotic course. I still feel the same even worse, I still have difficulty initiating urination, and i have to squeeze, some times I have bad smell to my urine, and i have white clouds in urine that looks like semen, I still have mild burning occasionally, I still have the numbness in my scrotum and penis, and I feel that I get aroused very quickly. sometimes I feel strange in my privates that I am going to cum just from touching and kissing my girlfriend, obviously the problem is worsening What is the problem, How to treat this?? I do not want to lose my fiancée because of this problem

• Well, the treatment depend sin the cause of urinary retention and causes include an obstruction in the urinary tract such as an enlarged prostate or bladder stones, infections that cause swelling or irritation, nerve problems that interfere with signals between the brain and the bladder, medications, constipation, urethral stricture, or a weak bladder muscle.

Tests to diagnose urinary retention include taking a urine sample, bladder scan, cystoscopy, X-ray and CT scan, a blood test for prostate-specific antigen (PSA), prostate fluid sample test, and urodynamic tests to measure the bladder's ability to empty steadily and completely.

• Treatment for urinary retention includes catheterization, treating prostate enlargement, and surgery and this should be discussed with your doctor after the right cause is found.
