What Causes Constant Bilateral Pulsatile Tinnitus?

I'm 50, 125 lbs. with constant bilateral pulsatile tinnitus. My mri and ct both normal. The pulsing sounds like it's coming from neck/base of skull region and gets louder if I tilt my head back and tighten my neck muscles. I have had cervical fusion in the past and have spondylitis

HI,welcome to HCM,
Bilateral pulsatile tinnitis should be carefully evaluated.since both CT and MRI in your case are normal,the probabilty of any vascular lesion is very low in your case.I recommend you to have a bilateral Carotid arterial doppler to evaluate any carotid artery disease.Any abberant vasculature in the neck post neck surgery could be the likely cause in your case.Consult an otolaryngologist with carotid doppler scan for further help.
