What causes chronic pain in the muscles and tendons?

HI, I have seen many different doctors and no help. 11 months with thoracic and lumbar back pain, despite physical therapy, rest, ice, heat, nsaids, injection, chiropractic care, accupunture. The mri said slight bulging disc but no surgery needed, but I do feel it s all the muscles and tendons. It hurts all the time, the worse is sitting on floor and trying to get up. Can you please advise. Thank you. Tina
