What Causes Chronic Insomnia, Anxiety And Heart Palpitation?

I am 42 years old male patient working in Saudi Arabia, I am a constant patient of varaious psyciartrict doctors. my problem started more than 20 years before related to my sleep, since 20 years i have bad quality of sleep, also i have problem of anxiety / heart palpitations / fear of public gatherings, for that i have been treated with various medicines like arpipirazole, paroxitine, duloxitne, flunxol etc, which are benfitial for some extent, now from 1 years whenever i sleep ( even sound sleep or low one) my head / ears become hot and have burning feeling in my head and also sharp pain, recently i visited one neurologist in Riyadh Hospital he has done sbrain CT and some blood test like thyroid and sugar, that came normal and priscribed me some medicines like pregadex / ipramax / amitriptyline tablets, which are not working eithier, he than told it is related to stress and menta l exertion, he advised me to have consultation with a good psyciartrict, I engineer and my work is at office, which involve sitting on PC for 8 to ten hours, My stated problem is effecting me a lot, when ever i tried to concentrate more on my work, the severiatyof this problem is becoming more. I want your advise for this Thanks
