The fact that this cough only comes on when you lay down raises the suspicion of a possible heart problem. However, heart problems would not cause a fever. What are your usual temperatures during these crisis? Your symptoms do not point to a specific and clear diagnostic option and more information is required in order to be able to do so and propose a treatment plan.
