What Causes Bruise On The Knee?

Hello, diabetic Mother (80yrs) tripped on raised sidewalk, hit knee/upper part of lower shin on concrete when fell 5 dyas ago. Bruise was at knee and upper shin, has diluted a liitle, but mostly bruise has traveled to lower shin and now upper foot, progressivlely traveling down the foot an inch daily. Do bruises travel, and so while lower leg is bruised front and back of calf, etc. The foot and lower leg were not bruised initially and now are and provide some pain. The fall was hard and so there has been numbness along with the pain. Little difficult to breathe laying down, but already was that way as she was sick with dizzying flu, and barely getting over that, but now has this. What would be good, recommendations? I put tiger balm and arnica on it daily, and it has diluted somewhat, but traveled downward moreso. Now, where there was no bruise, lower leg and foot, there is a bruise and is accompanied by normal bruise pains where bruise shows. Should the bruise have traveled down to the foot, and only the bottom of the foot and big toe and area are free of bruise, until tomorrow maybe, what would be good to do. Still has to walk daily, walks freely and ok without limp. What would be good, recommendations?
