hi
thanks for posting in HCM
I have understand your concern

probabilities of your symptoms are
1. chronic obstructive lung disease due to smoking
2.bronchial asthma
3.muscular pains leading to chest tightness
I can suggest you to get done
1.chest x-ray
2.pulmonary function tests
3.ECG

my advice is to stop smoking otherwise in long run you have to face many complications

after that results meet the physician , you may need bronchodilator therapy.
