What Causes Bleeding In Stool?

For two years, almost every time i use the restroom for #2 i have 5/10 pain on the anus and there is blood in my stool. The blood is bright red, my stool color appears normal other than that. I am 27, 96 pounds, 5"2. and last blood test 7 months ago was normal. Why is this happening? Thank you.
