Hi,

Usually postmenopausal bleeding to be considered as a condition to be evaluated as sometimes it may be due to some serious conditions. According to your description I am assuming that:

- The uterus is bulky, which indicates that the size of the uterus is more than the normal.
This can be seen in parous women. As such thickened or bulky uterus may not lead to a major problem. If the thickening of the uterus is because of conditions like adenomyosis then, that may lead to pain during periods or excessive menstrual flow in reproductive age women. What usually these conditions may not affect the menopausal women.

-If you mentioned about endometrium thickening, endometrium is the innermost layer of the uterus, usually that will get atrophy during the menopausal period, and It should be around 4 to 5 mm.
If the thickness of endometrium is more, you may need to get evaluated. Conditions like reproductive hormonal imbalance, rarely malignancies can lead to increased endometrial thickness in menopausal women. By doing endometrial biopsy the possible cause of the problem can be identified and treatment can be taken accordingly.

Take care. Hope I have answered your question. Let me know if I can assist you further.

Regards,
Dr. Sree Gouri S. R., OBGYN
