Noted the history of known case of migraines wherein Tryptans work in about 75 per cent of the times and if they do not then it is 24 hours of torture. Wants to know the reasons for black stools with persistent
vomiting...
The commonest cause of persistent vomiting is obviously
gastritis with or without an ulcer that causes bleeding which in turn causes black stool that is also called malena.
I would advise you the following:
Stop iron in any form.
Get the stool checked for routine and blood.
Upper GI
Endoscopy to see for the ulcers, H. pylori and so on.
Get started with a proper PPI like
Omeprazole.
Antacid liquid every three hours particularly when on stomach.
Soft bland diet made at home.
Avoid alcohol, tobacco in any form.
Watch for malena daily and if the bleeding persists, you will need therapeutic endoscopy to stop the bleeding and get a biopsy also.
Further treatment as per the findings and depends upon the condition that the patient is in.
