OBGYN, Maternal and Fetal Medicine 's  Response
Hi, I have gone through your question and understand your concerns. Fibroid in uterus can undergo red degeneration in pregnancy which causes lower abdominal pain, fever and backache.
Moreover, the spotting can be due to any haemorrhage around the foetus or low lying placenta.
I would suggest you to consult an OBGYN specialist to get properly examined, investigated and treated accordingly.
Hope you found the answer helpful.
Wishing you good health.
