What Causes Acne?

My son has very bad acne, and the dr. Said he could have some scarring. The dermatologist put him on a cream and antibiotic, but it is not helping. Now he wants to put him on acutane. He has been seeing a GI specialist the last year for some stomach pain. Dr isn't sure what is wrong, but with testing found his spleen to be enlarged and his gallbladder is only working about 10 percent. My questions are could the spleen be causing his acne. I may not have this right, but could the enlargement of the spleen, have something do with not having
enough white blood cells? If so, could there be a lack of enough white blood cell antibodies to kill off the bacteria in the area where the pimple starts (I know I didn't use the right term to discribe that). Also, I'm afraid of the side effects with acutane, and would like to know if you recommend anything else. One last question. Could acutane cause an enlarged spleen?

hi,
first of all acne vulgaris is due to multifactorial apthogenesis, the following four factors: (1) follicular epidermal hyperproliferation with subsequent plugging of the follicle, (2) excess sebum production, (3) the presence and activity of the commensal bacteria Propionibacterium acnes, and (4) inflammation.
Accutane (Isotretinoin) has lot of side effects, some of them quite severe. Some of the common side effects would be dry mouth, nose, skin, lips, itching or conjuctivis while the severe ones include bowel inflammation and pain, chest pain, decreased night vision, decreased tolerance to contact lenses, delay in wound healing, depression, fatigue, headache, nausea, peeling palms or soles, rash, skin infections, stomach and intestinal discomfort, sunburn-sensitive skin, thinning hair, urinary discomfort, vision problems, vomiting, etc
spleen enlargement is not seen with isotretinoin.
alternative drus for acne is Cap Minocyclin 100 mg once daily.
