What Causes Abnormal EKG Reading While Under Severe Stress?

Hello, I am just curious. My friend is under a lot of stress and had an ekg done. It was fine when he was lying down, but when he sat up the ekg read as if he was having a heart attack (according to his cardiologist). What would possibly cause this?

Hi,

He might be having coronary insufficiency. With stress and exertion, there is sympathetic outflow which increases heart rate and oxygen demand of heart which is not met. This causes the appearance of new EKG changes. Take him to a cardiologist for further workup which includes Echo, TMT and angiogram.
