What Causes A Painful Lump On The Foot?

. I have a lump the size of a half dollar on the top middle of my foot where it attaches to leg . It's painful to touch. I've had bloodwork,they thought gout , but it is not. I'm on blood thinners my INR is 3.5 it's not a clot . So what's going on?
