Internal Medicine Specialist 's  Response

Since it is painful, most likely lump is infective in origin and related to vertebral body possibly cold abscess.MRI spine (thoracolumbar) would be justified to determine the exact nature of the lesion.

Hope I have answered your query. Let me know if I can assist you further.

Take care
