As per your history please follow and answer my questions whether you are clinically depressed or not? 1 U do things slowly[Yes/No] 2 U feel your future seems hopeless[Yes/No] 3 It is hard for you to concentrate on reading[Yes/No] 4 U feel the pleasure and joy has gone out of your life [yes/no] 5 U have difficulty making decisions.[yes/no] 6 U have lost interest in aspects of life that used to be important to you[yes/no] 7 U feel sad, blue, and unhappy[yes/no] 8 U are agitated and keep moving around[yes/no] 9 U feel fatigued.[yes/no] 10 It takes great effort for you to do simple things[yes/no] 11 U feel that you are a guilty person who deserves to be punished[yes/no] 12 U feel like a failure[yes/no] 13 Do you feel lifeless -- more dead than alive[yes/no] 14 Your sleep has been disturbed -- too little, too much, or broken sleep[yes/no] 15 Do u spend time thinking about how you might kill yourself[yes/no] 16 Do u feel trapped or caught[yes/no] 17 Do you feel depressed even when good things happen to you[yes/no] 18 Without trying to diet, you have lost, or gained, weight.[yes/no] If you are clinically depressed then you must have greater than 5 positive signs.
