Two Circular Holes In Bottom Of Toe, With Pus Discharge And Swelling. Is It Gout?
I have a pt with a single toe that is swollen and hard to the touch. There are two circular holes on the bottom of the toe and when the toe is squeeze there is a white toothpaste consistancy discharge that comes out. There is no odor. His pcp says that it is just gout . I am concerned for this pt, and the toe is only getting worse.

From history it seems that he might have Osteomyelitis of toe.
Go for x-ray toe and find out the reason.
After diagnosis line of treatment can be decided.
Rule out diabetic foot as well.
Ok and take care.
