Tender, Aching Testicle, Difficulty Passing Urine, Recurring Lump. Reason?

Hi, My boyfriend has a painful left testicle and his symptoms are. - Tender, aching testicle, cannot pass urine easily (sometimes), slight lump but it sometimes comes and goes. He can find the lump easier in the shower. urine comes out white once in a while, sometimes after he pee's there is a slight discomfort between anus and scrotum these last 2 symptoms aren't very common but he has been experiencing them for years now, He doesn't have any other symptoms of prostate or testicular cancer. What is wrong with him?

He needs proper clinical evaluation and investigations.
Investigations include routine hemogram,RBS,urine RE/ME, Urine culture and sensitivity, Ultrasound of abdomen and scrotum,further investigations can be done if needed.
There may be various reasons for his symptoms.
Proper clinical evaluation and investigation reports can lead to diagnosis and treatment.
