Hello and welcome to HCM,

The presence of low grade fever, mild hepato-splenomegaly at 15 years of age is highly suggestive of infectious mononucleosis.
However, there are many other conditions which cause similar symptoms and signs.
Infectious mononucleosis is caused by epstein barr virus.
There are many other viral infections causing fever with hepato-splenomegaly.
These viruses are - CMV, rubella, etc.
Other infections are - leishmaniasis, malaria, schistiosomiasis, etc.
Malignant conditions can also lead to similar picture e.g. lymphoma, myeloproliferative disorders, etc.

Infections are more common than malignancies.
Monospot test is a test done for confirming infectious mononucleosis.
Consult your doctor for further investigations and thus management.

Thanks and take care
