You are known diabetic for so many years,you are over weight also apart from uncontrolled Fasting and Post prandial blood sugar level.
!80 mg/dl fasting and PPBS 280 mg/dl is high so your doctor rightly added combination of Metformin and sitagliptin which increases peripheral utilization of glucose and insulin sensitivity.
These medication should help you to have better control of diabetes, Macraberine is Vitamin B12 which is common deficiency in diabetes and causes peripheral neuropathy and even muscular pain which should help you.
In case blood sugar are not controlled by drugs and diet it is better to start Insulin therapy.
In case of back pain you should also see it is not due to spine problem like slip disc to rule out a MRI scan of spine should be done.
Please take care of diet, exercise and try to lose some weight.

With the available described symptoms, there appears to be possibility of partially controlled blood sugar with diabetic neuropathy.
Diabetes is a multi organ disease with involvement of almost all organs of body due to its complications.
Blood sugar both fasting and postprandial must be in controlled range.
Diabetes is due to progressive loss or destruction of beta cells in endocrinal part of pancreas.
When diabetes has been diagnosed, less than 20% beta cells remains in pancreas and progressive destruction of beta cells is still continuing.
You should consult with internal medicine specialist/ endocrinologist and should go for thorough neurological check up.
You should take proper treatment for control of sugar.
You should continue Macraberin and Istamet along with medications for diabetic neuropathy.
You should also take epalrestat, pregabalin/ gabapentin and oxcarbazepine.

You are suffering from uncontrolled diabetes and overweight.
There can be various causes for your back pain which includes arthritis,osteoporosis,dibetic neuropathy,obesity,myalgia may be due to use of anti cholesterol drugs etc.
You require a thorough investigations to look into the cause for your back pain.
