Taking Daonil, Metformin, Glimeperide, Januvia For Diabetes. Fasting And PP Levels Indicators Of Blood Sugar?

I am a diabetic for 10 years. I am aged 62. Only once my PP Blood Sugar level rose up to 305. Recently, Fasting BS
is higher than PP. The FBS is always around 150 while PPBS remains around 120 on medication or more up to 190.

Now I am on the following medications:

01. Daonil 5mg before breakfast.

02. Metformin 500gm SR + Glime peride 2mg before lunch and dinner.

03. Januvia 50 mg before dinner.

I tested Blood sugar after taking these new regimen after a weak. The picture was encouraging. yesterday, I
tested Blood Sugar around 4.00pm - tea time . It showed 193.

Are only FASTING and PP Levels the only indicators of Blood Sugar level? Am I on right medicines. My blood pressure is slightly high. I take Atenolol 25mg once a day.

Kindly help me with your opinion.

Hello,
Blood sugar indicators are fasting and PP sugar levels along with HBA1c level.
Your medications are all right at the moment but make sure to keep checking your sugar levels because as the age advances, oral diabetics gradually loose their efficacy. In such an event you have to change your medicines or doses of current medicines or starting injectable insulin.
ATENOLOL is a beta blocker and is avoided in diabetics.
You must switch to either ACE INHIBITORS or ARB's.Consult your doctor for this.
Your target BP should be below 120/80.
Keep your blood sugar levels, blood pressure levels and serum lipid profile in tight control because uncontrolled parameters lead to various types of micro as well as macro vascular complications.
Last but not the least, you must do daily aerobic exercises for 45 minutes a day along with proper dietary control.
Thanks

Hi,
Thanks for using Healthcaremagic,

After going through your history, you are having Diabetes type II and Hypertension and taking treatment for both.
Your Fasting blood sugar is 150 mg/dl it needs to be reduced further.
Please discuss with your treating physician regarding using long acting insulin at least once a day which may help to control the fluctuations in blood sugar level. You can always take a second opinion from Endocrinologist.

I presume blood pressure is well controlled with the drug you are using in presence of diabetes antihypertensive from ACE inhibitor group are commonly used.
Please also take care of your diet and regular physical exercise like walking 30 to 45 minutes daily will certainly help.
Hope this information helps you.

Take care.
Good Luck.

Diabetologist Dr. Pratap VGM's  Response
Dear sir/madam

The main aim in the treatment of diabetes patients is to avoid the chronic microvascular and macrovascular complications which can be achieved by good control of blood sugars

Usually if the second phase of insulin secretion is better than first then you will get good post prandial control than fasting.

Your current regimen is good but glibenclamide and glimeperide belong to same class, I think there is no need to take it both.

Thanks
