Suggest ways for conceiving after failed IUI and IVF procedures

Dear Doctor,

We were married 12 years back but till now we are without any issue. My present age is 45 years and age of my wife is 43 years at present. After one year of marriage i.e. in the year 2000, we consulted gynaecologist, who prescribed several courses of clomiphen. Injections like choragen, pregnyl etc were also administered. Apart from this medicines like letoval were also taken by my wife. But despite this, we were unable to have a baby. Subsequently, laparoscopy / EB was conducted in January 2002 and it was diagnosed that both tubes seen healthy and potent , spill and ovaries normal, pelvis clean. After laparoscopy, the courses of medicines was repeated. After, we consulted another doctor from Mumbai (Dr. Firruza parikh), who suggested cervical dilatation, which was done January 2004. Dilatation was possible upto Hg 8 with difficulty. After this we were asked to follow CC (Tab fertyl, Inj profassi, Inj IVFC 5000) + coital courses for 3 months. After failure of this course, another course for 2 months which include Inj Lupride, Inj IVF – M with ovulation monitoring followed by Inj IVF-C 10000 iu, IVF 5000 iu. After this a cyst was developed and course was stopped after two months.

After the failure of above course, we shifted to Delhi in April 2006 and consulted doctors there. First they asked us to go for HSG and the report of which was ‘Impression: Normal Study’. Another Dr. in Delhi performed Mycobacterium Tuberculosis and NK cells after which my wife has take 9 months medicinal course of AKT till Feb. 2007. After which Microbiology was again conducted at Appollo Hospital Delhi on 28.02.2007 with report ‘Negative for IS-6110 ‘Sequence of Mycobacterium Tuberculosis’ Conclusion: Endometrium *SECRETORY PHASE. After this two IUI course were done in March & April 2008 at Appollo Hospital Delhi. After failure of two IUI corses, IVF was done at Appollo Hospital in May-June 2008. After failure of IVF, we discontinued the medicines as my wife developed a cyst in left ovary.

Now keeping in view the above history, we may please be guided what steps should we take to have a baby.

An early response will be highly appreciated.

With regards,

As you both are above 40 yrs its better you decide to go for for in vitro fertilisation techniques.
Also its important that screen the embryo for any chromosomal abnormalities .

Hello. In January 2009 I was taking tbe medication for an IUI however my husband and I wanted to try to conceive on our own , but no luck. The second month we tried again but something was wrong with the medicatiom pen so i had to stop and wait to restart another cycle. So, the third month we did everything and went throuhht the compkeye IUI prcesure and I became pregnant. Unfortunately, I was having pains ans had to be rushes to the ER. It turned out that I had a left ectopic which ruptured (noone including the doctors knew about the ectopic). We all thought I was pregnant with only one fetus in the uterus . The one that the doctors had thought was the only one and in the correct location was actually a left cornual ectopic pregnancy , making it two in the left tube. I had emergwmcy surgery and was in the hospital for four days. I bleed internally which caused my lungs to partially collapse. By the time I was able to try again I had lost my health insurance. I now have health insurance which cobers IUI but not IVF . I will be 37yrs old in a couple of months and I do not want my clock to stop before Im blessed with my child/children. A few months back I spoke to my doctor again and he suggested the IVF due to what happened in 2009 and to be on the safe side of not another ectopic, specially with only one tube left. I know there is a chance for ectopic with either procedure. I am just a rollercoaster with this and i just need second+ professional opinion. Thank you in advance! Note: It took 5 yrs (saw same doc at clinic) before the pregnancy- i was only almost 11 wks pregnant

Sir, We have married before 10 years. In 2004 my wife got pregnant and after one week on further scanning it was identified as an ectopic and on the same day she undergone a laporoscopy to remove it . during surgery due to the bleeding left side tube was removed. Next year she conceived again in right tube(ectopic) and laproscopy milking of right tube with removal of corps luteal cyst . However in 2009 we tried with IVF but it also failed. In 2012 an HSG report said the right tube is not showing any block and we tried in natural way and IUI but no result till date. Now I am 42years old and my wife is on her 33. We hope a good advice from you.(except IVF). Regards. Satheesan

I am 33 yrs old woman, 5 2 n 78Kgs. I had an ectopic pregnancy in my right tube which was removed through medications 4yrs ago. Then had to remove my right tube due to hydrosalpinx 3 yrs ago. After this I had conceived through follicular monitoring n stimulation but it turned out to be weakly positive followed by an failed IUI. My doubt is what happens to the eggs released in my right ovary as there is no tube for it to travel down. I have always found that my right ovary responds to medicines better than left. Should I try for an IUI again or should go for an IVF. I also have an history of PCOD due to which my periods are irregular.

My husband has been on Treatment to raise his T levels. First he used Testim gel for several months, now he is using injections, which he gives himself at home. My concern is we would like to conceive, and we know these treatments can lower sperm count. He is reluctant to do a Semen analysis, but I think it may be the only way to know if conception is possible. The Urologist he is using has suggested it, and also wants him to start HCG which can raise his sperm count. All very confusing, can you enlighten me?
