Suggest Treatment For Vitiligo In A Child

My kid (girl) of age 8 years have early signs of apparent vitiligo. She has small whitish marks near her right eye, at the lower side outer corner, and some more light white marks at left eye, lower side, outer corner. Another little white mark is there at her left waist. She was advised Mometasone cream, tacrolimous ointment, L.D. lotion (and folowed by sun exposure for 10 mins), and melgain lotion at night. Things improved on both the eyes, after a 45 days usage of the medicines. The eyes were almost patches free, and the waist mark was also decreasing. Doctor advised to stop using near the eyes, since they were getting a bit darkish. However now, after stoppage of a month, little white marks are again appearing on her eyes. The waist mark is continuing to decrease. i am worried due to this recurrence in the eyes. Rest of the body, i have monitored, and has no more marks.The marks appeared 1st time in Jan/Feb this year.

Hello and Welcome to ‘Ask A Doctor’ service. I have reviewed your query and here is my advice.

Recurrence is a distressing yet expected issue in vitiligo. I suggest you to restart with tacrolimus ointment for the patches on her face. Use it twice daily. Rub it in. At night i suggest you to use fluticasone propionate 0.05% cream which is a milder and safer steroid unlike mometasone furoate, specially when used on face. I think that should be sufficient treatment for her facial vitiligo. Treatment may need to be continued for 1-2 months before depigmentation starts to appear. For other body areas too you may also use the same treatment i.e tacrolimus and fluticasone.
