Suggest Treatment For Trace Of Protein In Urine

I am 85 y.o. male in excellent health 5 9 132 lbs exercise almost continuously in home gym. I recently did a dipstick and have trace proteinuria, otherwise all labs are WNL. I am retired MD and have the relevant labs with GFR by creatinine and cystatin of 70ml/min etc and 77 . Creatinine is 0.99 cystatin is 0.68 mg/L. Do I need to do more than eliminate the excess whey protein that I was taking for workouts and go on a relatively low protein diet mainly of egg whites and vegetables.

Thanks for your question.

After going through your question, I can understand your concern.

There are many different causes for presence traces of protein in urine like -
- Fever.
- Exercise.
- Exertion.
- Urinary infection.
- Diabetes.
- Hypertension (high blood pressure).

If there is no diabetes, Hypertension, fever you can ignore the finding, there is no need of any further evaluation as all other Kidney parameters are acceptable considering your age.
You need not alter your current protein intake.

If there is presence of diabetes or Hypertension then it may be indicator for need for further evaluation.
In that case following investigations may be advisable -
- Fundoascopy to rule out diabetic and hypertensive retinopathy.
- 24 hour urine albumin /protein excretion or early morning spot urine albumin creatinine ratio.
- Ultrasonography of abdomen and pelvis for Kidney size, shape, echotexture.
