I understood your health concerns and feel Concerned about them.
Based on the facts of your query the reply of your query is-
Your current health issues seem to be due-Improper pubertal development of the testes and its travel to bottom of scrotal bag,as in normals.
But with a history of ?auto-reversed Torsion attack of testes,
testicular dysfunction with low
testosterone levels, could be the cause of the immature descent of testes and or Strong Cremasteric muscle-Reflex, still by this age of 27 and is a cause of Concern medically and needs to be investigated and treated soon by Consults with
Urologist and Surgeon.
Options available to YOur problem-
They would assess the cause of the recurrent retractile testes at this late age, but with possibility of total descent to bottom of testes.This could be some Congenital pouches with fibrous adhesions, which could trigger strong cremasteric reflux at this late 27 yrs, due to which the left testes is retracting in your case.
In this background- you may need corrective surgery-of Orchiopexy- at hands of Urologist and / or Surgeon, whereby the testes is brought and fixed to bottom of Scrotal bag, to save the sex hormone producing function.
I would suggest
Second opinion with Urologist who would get the USG with Colour doppler, to assess the cause of the retractile testes and would protect your testicular function, which seems to be near normal ?or hampered little bit by the ?Torsion in your childhood.
This also could be due to the Congenital shortening of the Vas deference / Vessels with accompanied hidden-left
Varicocele, as the testicular discomfort in your case increases after retraction, compressing more on the Engorged veins which are un-diagnosed still.
You should not worry as your testicular function is still near normal, but need to act fast as suggested above
Hence Other causes-as cited above- need to be ruled out,by verification with Surgeon and Urologist.
Do's-
Consult with Urologist and Surgeon who would investigate physically and by USG with Colour Doppler study if needed.
This would fix the cause amongst the above or other possibilities of discomfort after repeated retraction lately in your case.
You need to treat it with Surgeon/ Urologist which should be done on priority and urgently to avoid further complications.
This would resolve the complaints of your's.
Constant followup with local family doctor with specialist Urologist / Surgeon-for long term would give you good results in time to come,which would depend on meticulous followup treatment of your health concerns,as the case treatment is complex to assess and execute.
