General Surgeon 's  Response
Hi.
Thanks for your query.
Since this is a known case of varicocele with symptoms of pain and so I wold advise you the following:
Get the semen analysis.
Color doppler with ultrasonography
Consult a General Surgeon or an Urologist and get operated for better results.
Medicines do not help much unless there is an infection.
Till surgery is done you may use the medicines.
Have scrotal support.
Avoid tight underwear and pants
Avoid exposure to hot and humid air.
I find this answer helpful
