Suggest Treatment For Swollen And Painful Thumb After Accidentally Jamming It

Hello,
I smashed my thumb in the car door Monday night. I had to open my car door to get my thumb out. This happened three days ago. I've been icing it for 20 minutes, every four hours. It's still swollen pretty bad. I have a splint on, but the throbbing/burning pain is still the same. I was told to follow up after 3 days...should I follow up? I work in an office setting...maybe from me moving my thumb? Not sure what to do. I have a hard time sleeping!
