General & Family Physician 's  Response
Hi & Welcome.
Based on the history given you seem to be suffering from congestive heart failure.
You are advised to consult a physician/cardiologist for proper evaluation and certain tests like ECG, Chest X ray, CBC, Echocardiography, etc to assess the severity of the disease.

Your doctor will prescribed drugs like angiotensin converting enzyme inhibitor (Lisinopril) and a loop diuretic ("water pill") for the treatment of such condition. Beside the underlying condition like high BP, ischemic heart disease, or diabetes must also be treated.

Hope this will help.
