Suggest Treatment For Sharp Pain In Arm

I am a physical therapist. I am having sharp pain under my left scapula with difficulty also raising my arm. I started with bicepital tendon pain at the left shoulder in the groove. I am also having trouble taking a deep breathe. When I try to slide my should anterior and posterior to have my scapula glide there is a loud clicking noise. I have tried moist heat and ice without relief as the pain is deep. Any ideas?
